ELEV8 Consulting Pty Ltd trading as ELEV8 Wellness and Health takes all reasonable steps to protect personal information from misuse, loss, unauthorised access, modification, or disclosure. This includes implementing appropriate physical, electronic, and managerial security measures.

Cancellation Policy
If the participant makes a short-notice cancellation, which is less than 48 hours before any planned Service episode (Private clients and NDIS participants), the provider will charge 100% of the agreed price for the cancelled appointment. This fee may be charged against a participant NDIS Plan up to 8 times per year. A short notice cancellation also includes no shows where the participant or client is not present greater than 10 minutes of the intended Service/s commencement time.
For other cancellations, where the participant and clients have provided notice of cancellation prior to 48 hours before the scheduled service, the provider will not charge a cancellation fee. If the participant can show evidence of a medical emergency (e.g. ambulance receipt or discharge summary) then a cancellation will not be charged. Forgetting or making other appointments at an agreed/planned Service time is NOT a valid reason.
Where a participant fails, at short-notice or without notice (includes no-shows), to keep the scheduled support, the provider will make reasonable effort to contact the participant to determine if there is an additional problem. If the participant is late and provides notice the provider will reduce the total time of the actual service in order to maintain its other customer schedules – however the total time of the service is still payable (waiting/absence time plus shorter service delivery time).
The provider may also charge a short notice cancellation for any reason where it is NOT safe to undertake Service/s as intended including due to behaviours of the participant or others, environmental or circumstantial matters, or unadvised illness at the participant's residence.
Where there is a specific risk that a participant will frequently make short-notice cancellations for a support due to the nature of a person’s disability or the nature of the support (e.g. Employment Services), the provider will make individual arrangements to minimise the number of cancellations occurring, or may choose to end the Service/s if cancellations cannot be reasonably managed, or 8 cancellations per 12 months is reached.
